Registration Waiver Form

I affirm that I am the parent or legal guardian of the participant(s) named below, or that I have received authorization from the parent or legal guardian to sign this agreement on their behalf. I agree that both the participant(s) and I will comply with all applicable terms, posted safety notices, rules, and verbal instructions as conditions of participation in any party and/or program at The Playroom Cafe. *

I agree to promptly notify an employee or representative of The Playroom Cafe if I become aware of any hazardous condition. I understand and acknowledge that I am responsible for my child’s safety and behavior at all times during each visit.*

I am aware of the risks of participating in play at The Playroom Café. I, on behalf of myself and the participants listed above, knowingly and freely assume all such risk, both known and unknown, including those that may arise out of the negligence of other participants.*

I agree not to hold The Playroom Café LLC, its officers, agents, or employees liable, or pursue legal action against them. This waiver absolves The Playroom Café LLC of any duty for injuries sustained on the premises before to, during, or after the activity. By signing this agreement, I agree to hold The Playroom Café LLC completely harmless, including financial responsibility for any injuries sustained, regardless of the cause or circumstances.*
